Our office is called Heart of Iowa Community Services. We act as an entry point to a regional system of services and supports by taking your Funding Application form, determining your eligibility for county funding, evaluating your needs, and working to create and implement your individualized plan. If we cannot fund a service or support for you, we will refer you to an agency that may be able to help you. If you need a service or support that is not in our county management plan, we will work toward finding a provider to provide that service or support in our community so that it may be available to you in the future.
HICS provides leadership and management at the local level for designing a regional system of care for Mental Health and Disability Services. The design of the system is based on the expectation that individuals and families will have multi-occurring issues of all kinds, and will incorporate an organized quality improvement partnership process to achieve the vision defined at the beginning of this Plan.
We believe in service participant empowerment. It is essential that individuals have freedom of choice, and take an active role in deciding what services and supports you need and how those services are to be delivered.
